What is Heart Support?
Heart Support  is a medical program that provides care to advanced cardiac patients
in the home. Heart Support care is focused on controlling symptoms, such as pain, shortness of breath,
weakness, nausea, fatigue, anxiety and depression. Heart Support brings all the needed people
and tools -  nurses, home health aides, family services, spiritual care, volunteers, medications,
equipment -  to you at home when you need it. One of goals of Heart Support is to allow you to stay
home and out of the hospital when at all possible. Our care is available 24 hours a day to come to
you and help you manage changes and crisis. Heart Support cares for the entire family, giving
needed support and education to everyone involved.
Who Provides Heart Support Services?
Heart Support is a program of Sangre de Cristo Hospice and Palliative Care, a
non-profit agency that is the expert source of palliative care in our area.
What are the costs?
Heart Support is fully covered by Medicare, Medicaid and most private insurances.
Who is Eligible for Heart Support?
Heart Support is for people with advanced heart disease whose symptoms are no
longer controlled through medications or whose course is complicated
by other medical factors. People come to Heart Support when their main focus is
controlling their symptoms. All residents of o
ur coverage areas are eligible,
including residents of skilled nursing facilities.
Can I Still See my Doctors?
Your doctor continues to manage your care while on Heart Support and you can still meet with your doctor and your cardiologists as
needed. Tests and treatments that are needed to keep your symptoms under control can be continued and are encouraged.
How do I Start Heart Support Services?
All it takes is a phone call. Anyone can call and request more information. We can sit down with you and answer all your questions
and find out what you need. When you elect Heart Support services, we can handle the admission process with your doctor.
When do I call Heart Support?

The best time to call Heart Support is before you need us.
We can meet with you and your family and explain our
services in full. If the time is not right for Heart Support,
we will put you in touch with the services that are right
for you. Then you can bring Heart Support in when the
time is right. Discuss your options with your doctor and feel
free to call us with questions. Heart Disease is hard
enough to manage -  you don’t have to do it alone.
